ACP First Steps Facilitator Certification/Recertification Course (*Rescheduled to September 2022*)

This Advance Care Planning (ACP) Facilitator Certification course is for those who assist individuals ages 18+ in having thoughtful conversations about the care they would want in the event of medical crisis. This training provides the skills and expertise to lead conversations on future health care wishes and preferences.
Core Course Goals:
- Identify intended populations for advance care planning conversations.
- Verbalize strategies to engage individuals and healthcare agents in advance care planning.
- Demonstrate communication skills to promote person-centered conversations.
- Apply critical thinking to discern what matters most to individuals.
Intensive Course Goals:
- Define benefits of First Steps conversation and identify appropriate target population.
- Demonstrate the application of the person-centered decision-making framework to elicit preferences and decisions.
- Create strategies to transfer individual goals, values, and preferences to a written plan.
- Demonstrate beginning competency in facilitating First Steps ACP conversations through role-play exercises.
Pre-requisite: There are four online modules to be completed prior to the first training session. Instructions to complete the modules will be emailed to participants approximately one month prior to the first session. The modules should be started as early as possible to allow adequate time for completion. In addition to the online pre-training modules, all three training sessions must be successfully completed to earn certification.

Audience: This training is intended for HCV partner organizations and HCV Community Facilitator volunteers. Please contact us before registering for this training.
Cost: $330 for full certification enrollment. Includes all three classroom/virtual training sessions, online training modules (completed prior to classroom sessions), training materials, refreshments, and CEUs. * Virginia law requires that community members who are not attorneys or health care providers must complete a certified training program to assist with creation of advance medical directives. Honoring Choices Virginia has been certified by the Virginia Department of Health to provide training to community members.
