At Honoring Choices Virginia, we envision a world where every Virginian has access to high quality advance care planning that is honored in the delivery of care. We believe that access and person-centered care are critical to eliminating health disparities and distrust of the medical field. We value listening, consensus, engagement, collaboration, consistency, and equity. Our spaces are inclusive and value-neutral.

Our partners form a diverse network of organizations–including faith and cultural communities, health care systems, government agencies, nonprofit organizations, businesses, and private practices–all committed to providing inclusive access to high quality advance care planning and honoring choices in the delivery of care. Honoring Choices Virginia welcomes and supports any organization or group that wishes to become involved in this work. At HCV, no two partnerships look the same.
