ACP Professional Learning Session – Different ADs for Person Centered Care (05.21.2025)

Our advance care planning (ACP) professional learning session is a training event designed to educate ACP/AD professionals about the principles, processes, and best practices related to advance care planning. Advance care planning involves discussions between individuals, their families, and healthcare and other providers to establish preferences for future medical care in the event that they are unable to communicate their wishes themselves.
